New Patient Registration

We have a registration pack available at reception and our receptionists will assist you with anything you are unsure about. Our catchment area for the practice is G41 post code only. It would be most helpful for you to have a form of identification with you.

New patients are always offered an appointment with the doctor or practice nurse, for an initial health assessment as well as an explanation of the services we provide.

You will be registered with the Practice, rather than an individual doctor but you can choose to see one particular doctor if you wish, although it may mean a longer wait for an appointment.

Your Contact Details

Please help us to keep in contact with you by ensuring you inform us of any change of telephone/mobile numbers or change of ad-dress Should you move house out of our area please remember to change your GP – please contact the Practitioner Services who will be able to help and advise what surgeries are near you should you have any difficulty finding a new GP. You can also visit NHS Choices on the internet for helpful information.
